Empowering and engaging families in the process of care is a top priority for Nemours Childrens Hospital.

Technologies Focused On Patient-Centered Care – As partners in care delivery, patient families will have secure and confidential access to clinical information wherever they may be, enhancing communication and follow-up with patients prior to coming to the hospital and after discharge through the use of personal health records systems.

For several years Nemours had been investigation how to better engage patients and families in the process of care. Recognizing the evolving needs and technological capabilities of patients and their families, a consumer health platform to support their corporate strategy was being sought. The following key requirements were developed through extensive family interviews: efficient and effective communication between families of patients and clinicians; ability to track key parameters for chronic and complex patients; and the ability to view lab results and appointments. Clarity Health Journal is the first product to embody these needs.

The 2011 Cluster Grant award for Obesity Research Cluster (ORC) calls for implementation of Clarity Health Journal. Nemours seeks to better understand the utility and effectiveness of decision support messages delivered directly to the patient. Clarity Health Journal delivers a wide array of features, otherwise not available to patients at Nemours. It will also integrate with the current portal known as MyNemours.

Clarity Health Journal was chosen because the application delivers a comprehensive web 2.0 experience to patients including social networking and the ability to deliver messages via mobile devices. The project will engage families dealing with Child Obesity in order to better understand the use of technology to improve patient outcomes by empowering families.

The Clarity Health Journal designed as a family centric health application allows for information sharing and monitoring. It will be used by family members of patients at Nemours. Focusing on both the patient and the family is a unique approach to healthcare. Parents of children with health conditions are motivated in monitoring their health and working with the care team to foster health and wellness while lowering the cost of healthcare. The Clarity Health Journal integrates the hospital record and the doctors records along with personal health details to create a more complete health record.

This pilot program will look at outcome measures related to patient compliance using a combination of a food and an exercise diary. Dr. David Milov (CMIO of Nemours Childrens Hospital). Looking at how the family involvement can enhance the quality of health care, Nemours is working hand-in-hand with families to find innovative ways to help manage childrens health. Dr. David Milov indicated this project will be great for patient communication, improve patient care, decrease missed appointments, decrease misunderstandings when it comes to patient instructions, encourage patient/family to have responsibility for their own care.

Over the course of the pilot, Nemours will monitor early indicators of success with researchers and family focus groups.

Designed with both personal wellness and the efficient delivery of healthcare services in mind, Clarity Health Journal fosters better quality health by promoting family and patient participation in the management of their health, integrating health data into the healthcare providers work flow, increasing the quality and accuracy of that information and driving down the cost of healthcare, explains Saverio Rinaldi, President and CEO of Consulting Cadre International Inc. Clarity Health Journal helps improve communication by involving the family in the patients care in and outside of the hospital; we feel this will reduce readmissions and emergency visits.

Clarity Health Journal has the capability to cover the broad spectrum of individual health management, from post-natal and elderly care to normal health and chronic disease management.

The solution takes an individuals health information, which today can be found in paper and/or electronic form in one or more places, including general practitioners and/or specialists offices, hospitals, clinics, long-term care facilities and patients residences and re-engineers them into an accessible, updatable, electronic health journal.

Clarity Health Journal patient-managed information provides seamless integration between the individual, doctor, hospital, clinic and government, ultimately resulting in more accurate health diagnosis and better personal care, adds Mr. Rinaldi.

The stunning Stafford Residence, a 5,000 square foot home in Breckenridge, Colorado designed by Michael F. Gallagher, Architect, recently won 8 awards in the Summit County Builders Association Parade of Homes.

In the 5,001-6,000 square feet category, Michael F. Gallagher, Architects Stafford Residence was awarded first place in Exterior Design and Elevation, Best Kitchen, Best Master Bedroom Suite and Best Home. In addition, Alan Evans of Elevation Building Group took first place in Builder Concept & Workmanship for his construction work on the home and NorthPoint Design won first place in their category for both Interior Furnishings and Interior Finishes.

The home also won Best Overall Home for the entire contest, regardless of square footage. The Best Overall Home award was presented by the Summit Association of Realtors.

Situated high atop a ridge overlooking the Breckenridge, Colorado Golf Club, with endless views extending to the entire Ten Mile Range, this spectacular custom home was designed to take full advantage of its dramatic 2.2-acre site. Architect Michael F. Gallagher worked closely with the owners to design a home on the steep lot that feels completely level and accessible. The open floor plan of this Energy Star rated home highlights the views of the Ten Mile Range, Peak 8 and the top of the Imperial Lift at Breckenridge Ski Resort. The entire living quarters are on one level, with the upper floors designed to accommodate guests, children and future grandchildren. Adjacent homes are virtually invisible from every room in the home.

Maximizing the mountain homes southern exposure, Michael F. Gallagher, Architect designed a 350 square-foot outdoor living room that the owners plan to use all year long. Accessed through 9-foot by 9-foot doors in the great room, the outdoor living room faces due south and is protected from the wind. The floor is made up of stained and stamped concrete, while a fire pit and overhead heating provide warmth for year-round use. A state-of-the-art barbecue grill is built into the stone retaining wall. The room is covered by 14 feet of roof, while the adjacent hot tub was purposely left open to the sky for stargazing.

The Next Generation great room concept spans 3 levels, encompassing the living room, kitchen, dining, family and game rooms all the way up to third level lofts. A built-in sound system and home theater are enhanced by Acoustiblock soundproofing, preventing sound transmission between levels.

Fertilization occurs when a sperm enters an egg to form a zygote. The in vitro fertilization process begins when the eggs are retrieved from the womans ovaries by a reproductive endocrinologist. The eggs are deposited in a sterile petri dish and taken to the IVF lab. The embryologist examines the eggs under the microscope. This is what he sees. Each dark group of cells is a cumulous-oocyte complex. Here you can see the oocyte at the center. On the oocytes outer membrane, presence of a polar body indicates the egg is mature and ready for fertilization. Outside of the oocyte are the corona radiata cells, surrounded by the cumulus cells and the outermost granulosa cells. Here is the sperm sample, which has been washed to capacitate the sperm. Only these active, capacitated sperm can fertilize an egg. In the standard-culture form of IVF, about 150000 capacitated sperm are added to the petri dish of mature oocytes and allowed to incubate. 16 to 20 hours later, we can see that breakdown of the granulosa cells has occurred. See how dispersed they are? The embryologist removes excess cells so that he can look for pronucleii. Presence of pronucleii in an oocyte indicates that the union of sperm and egg—fertilization–has taken place.

They wanted to effectively make energy improvements to the home that is planned to house eight adult women and a state of the art learning center. The existing home, built around 1980, was assessed and modeled to require attic insulation, a new home heating/cooling system, lighting upgrades and a water heater as well as reconfiguration of the garage they planned to use as the learning center. A scope of work was developed that included insulation to R38 and weatherization, a requirement in the Energy Upgrade California program. A Hydronic furnace, a solar thermal/tankless solution to address hot water consumption for eight adults every day, photovoltaic/solar system, to address the anticipated increased electric usage and a mini-split heat/AC solution for the learning center was included.

He has provided project management and lead technical projects for Campbells, GMAC, GSK and PPA. In addition, David has provided account management and strategy, delivering large ecommerce solutions for consumer national brands including Ace Hardware, Rockport and Cost Plus World Market. David has developed open-source web and mobile strategies for B2B technology and healthcare companies including BioClinica, Vocollect, IMS Health and Vertex, Inc.

Prior to Metter Interactive David held senior leadership positions at Garfield Group, TrueAction (previously known as gsi interactive), G2i (previously known as Refinery) and Imirage, Inc.
